Job Description:
A company is looking for a Remote Care Coordinator to engage and
enroll members in a healthcare program.
Key Responsibilities
Telephonically engage and enroll members in the Mom and Baby
Beginnings program
Conduct health and psychosocial surveys while considering cultural
and linguistic needs
Verify member eligibility and conduct initial screenings and needs
assessments
Required Qualifications
Diploma or Associates in a healthcare-related field or equivalent
relevant work experience
A minimum of three (3) years of experience in a healthcare
field
Ability to work Monday to Friday from 8am to 5pm
