Job Description:
A company is looking for a Community Health Worker.
Key Responsibilities
Provide culturally appropriate complex care management to patients
in a primary care setting
Assist patients in overcoming barriers and navigating the
healthcare system
Support patients in making meaningful progress toward
health-related goals over a 6-month period
Required Qualifications, Training, and Education
High school diploma or GED preferred
Valid driver's license and clean driving record required if driving
is necessary for the role
Two years of experience in a social service or healthcare setting
preferred
Must be a resident of the community served with knowledge of local
resources
Basic computer skills preferred
