Coordination.
Key Responsibilities
Deliver exceptional service to members and care team through
various communication channels
Manage daily assignments to support the Care Coordination team with
care management needs
Schedule appointments, process referrals, collect medical records,
and maintain accurate case documentation
Qualifications Required for Position
2+ years of college education
2+ years of Medical Assistant experience
High school diploma required
Customer service experience in a healthcare setting required
Prior Authorization and Referral experience required
