Job Description:
A company is looking for a Case Manager to provide
member-centric, evidence-based care in a remote setting.
Key Responsibilities
Conduct comprehensive assessments and develop individualized care
plans
Implement and monitor care plans while coordinating services across
the healthcare continuum
Collaborate with members, families, caregivers, providers, and
interdisciplinary teams to deliver holistic care
Required Qualifications
Master's degree in Social Work, Psychology, Counseling, or a
related field
5+ years of clinical experience focused on mental health and
substance use
Experience with vulnerable populations in clinical, home care, or
telephonic environments
Current, unrestricted advanced practice Social Work
license/certification in the state of residence
Proficiency in electronic health records and care management
software
